Holiday Request Form

Attendance Guidelines and holiday request form.  Please print off the holiday request form and return to the School Office.  It is normal practice for Mrs Langson to meet with Parents and Carers to discuss the holiday request.

  Our most recent School Prospectus is attached in two pdfs.  The prospectus is so large and due to the restrictions of our website, we have had to split it in half, so please make sure you read both parts.

Uniform Order Form

School Uniform Order Form, please print off and complete and return to the School Office with the correct money or cheque (payable to Uphill Primary School).  When the School Uniform is ready for collection or note will go in the Office window.
